Self-declaration that there are no grounds for exclusion according to Section 123, Section 124 GWB and Section 2 WRegG.
-Proof of at least two reference projects in experience of supplying medical devices for usage in hospitals/clinics within the last three years 2022, 2023 and 2024.- Proof of a valid ISO 9001:2015 or Proof of a valid ISO 13485:2016 certifcate- The bidder must have an officially authorized after-sales technical service team as local representative in Ukraine. It must be supported by official documentation like an official agreement, or a letter of intent signed between the bidder and the local representative in Ukraine. It shall also include the address and contact details of the representative in Ukraine. The representative shall be able to offer warranty services, maintenance, trouble shooting and calibration services.
The execution conditions result from the information given in the contract notice and the tender documents.
